ADMISSION POLICY

Non-Discrimination Policy

Our Lady of Perpetual Help School admits students of any race, color, racial or ethnic origin to all the rights, privileges, programs and activities generally accorded or made available to students at the school. The school does not discriminate on the basis of race, color, racial or ethnic origin, administration of its educational policies, admission policies, scholarship and loan programs, and athletic and other school administered programs.

Our Lady of Perpetual Help School does not discriminate against any applicant or employee because of sex; and does not discriminate any applicant or pupil because of sex in admissions, educational program and activities.

CERTIFICATE REQUIREMENTS FOR NEW STUDENTS:

• a completed application form
• current report card
• standardized test results
• a Baptismal certificate (for Roman Catholics only)
• immunization records
• physical examination report (Kinder and First Grade)

GENERAL REQUIREMENTS FOR NEW STUDENTS:
• Students entering kindergarten must be 5 years old by September 1st: NO EXCEPTIONS
• Students entering 1st grade must be 6 years old by September 1st: NO EXCEPTIONS
• Students must be academically able to function successfully in the Our Lady of Perpetual Help School program.
• Students must exhibit social conduct consistent with the Our Lady of Perpetual Help School program.

GENERAL REQUIREMENTS FOR NEW PARENTS:
• Willingness to support Our Lady of Perpetual Help School’s policies
• Faithful, timely payment of all tuition and fees
• Financial support and involvement in the school’s primary fundraisers: Jog-a-thon and Auction Dinner/Dance.

AS OPENINGS OCCUR, STUDENTS ARE ADMITTED TO OUR LADY OF PERPETUAL HELP BASED ON THE FOLLOWING:
• All certificates and general requirements are fulfilled (as previously stated)
• Siblings of enrolled family members
• Registered members of Our Lady of Perpetual Help Church
• Catholics outside the parish
• Non-Catholics
