standardized test results
a Baptismal certificate (for Roman Catholics only)
immunization records
physical examination report (Kinder and First Grade)
GENERAL REQUIREMENTS FOR NEW STUDENTS:
Students entering kindergarten must be 5 years old by September 1st: NO
EXCEPTIONS
Students entering 1st grade must be 6 years old by September 1st: NO EXCEPTIONS
Students must be academically able to function successfully in the Our Lady of
Perpetual Help School program.
Students must exhibit social conduct consistent with the Our Lady of Perpetual
Help School program.
GENERAL REQUIREMENTS FOR NEW PARENTS:
Willingness to support Our Lady of Perpetual Help Schools policies
Faithful, timely payment of all tuition and fees
Financial support and involvement in the schools primary fundraisers:
Jog-a-thon and Auction Dinner/Dance.
AS OPENINGS OCCUR, STUDENTS ARE ADMITTED TO OUR LADY OF PERPETUAL HELP BASED ON
THE FOLLOWING:
All certificates and general requirements are fulfilled (as previously stated)
Siblings of enrolled family members
Registered members of Our Lady of Perpetual Help Church
Catholics outside the parish
Non-Catholics
